Volunteer Photo Agreement Policy

I hereby grant to the McCormick Care (formerly the Women’s Christian Association) and the McCormick Care Foundation the absolute right and permission to use my name, voice, likeness, photograph, and/or statements without remuneration, in any geographic jurisdiction, at any time and from time to time, for the purpose of advertising or promoting the McCormick Care Group, the Foundation, McCormick Home and/or McCormick Dementia Services and any other lawful purpose whatsoever, in any manner of media, to the extant permitted by law.

I hereby release and discharge the McCormick Care Group and the Foundation and their officers, directors, employees, representatives and agents from any and all claims whatsoever which I, and my heirs, estate trustee or other personal legal representatives or assigns, have or may have arising out of the use by the McCormick Care Group and/or the McCormick Care Foundation of my name, voice, likeness, photograph, and/or statements as provided herein.
